Yes. If your patients replace their phone mid Sensor wear, it’s important to log back into their FreeStyle LibreLink◊ account on the new phone with the same account credentials they used to activate their Sensor. Once they are logged back into their account, they should scan the Sensor to transition it to the new phone.
